Regulations regarding medications or procedures provided at school
The information provided outlines the procedures and requirements for students who need to take prescription medication or undergo procedures during school hours at LCWM. Here are the key points:
Consent Form for Administration of Medication/Procedure:
Parents are required to complete the Consent Form for Administration of Medication/Procedure during the School Day if their child needs to take prescription medication or undergo a procedure at school.
Parent authorization and doctor's orders must be renewed annually or if there are changes or discontinuation of medication.
Availability of Form:
The form is available in the health office or on the Health Services Website under Medication Forms and Guidelines.
Types of Medications/Procedures Covered:
The completed form is required for prescription medications, self-carry medications, procedures, and over-the-counter medications.
Self-Carry Medication Responsibilities:
Students needing to self-carry medication, such as Epi-Pen or Inhaler, must have a signed medication order by a parent, doctor, and student.
The student's ability to carry medication is at the discretion of the school nurse, and the student must prove they are responsible enough for this responsibility.
Secondary students (9-12) may self-carry over-the-counter pain medications (tylenol or ibuprofen) under the discretion of the Licensed School Nurse (LSN). The form must be turned in, or the student must keep medications in the Health Office.
Medication Container Requirements:
Medications must be provided in an original, currently dated, labeled container that matches the physician's orders.
An adult must bring any medications to the school office where it will be stored in a locked cabinet.
Record Keeping:
Electronic records will be kept on the medications taken by each child.
Disposal of Medications:
Any medications not picked up at the end of the school year will be disposed of by the School Nurse per guidelines.
Dispensing Medications:
Medications will be given to students based on instructions by trained school personnel.
